Telemedicine App Development Cost: What Drives the Price
The features, integrations and compliance work that decide what a telemedicine app costs, and how to scope a first version.
There is no single price for a telemedicine app. Two products that both "do video visits" can differ widely in cost, because the work sits in the features around the video call, the systems it connects to and the privacy rules it must follow. This guide explains each cost driver so you can scope a first version with confidence.
Core features
A basic telemedicine app usually includes patient registration, provider profiles, appointment booking, a secure video visit, visit notes, and notifications. Each additional feature adds design, build and testing time. Common additions are:
- in-app chat and file sharing between visits
- payments, insurance details and receipts
- e-prescribing
- waiting rooms and multi-provider scheduling
- interpreters or family members joining a call
Start by choosing one care pathway, such as follow-up visits for one specialty, and build only what that pathway needs.
Video
Most teams use a video provider such as Twilio, Vonage or Daily rather than building video from scratch. The provider charges per participant minute, which is a running cost to plan for. Build cost depends on how much you customize the call experience, recording rules and support for poor connections.
Integrations
Integration is often the largest single cost driver. Connecting to an EHR such as Epic, Oracle Health (Cerner) or athenahealth means working with their APIs, approval processes and test environments. The HL7 FHIR standard has made this easier, but every EHR has its own rules. Our EHR and FHIR integration work usually covers patients, appointments, observations and medications. Labs, pharmacies and payment systems add further integrations.
Compliance and security
If you serve patients in the United States, the app must meet HIPAA Security Rule safeguards: access control, encryption, audit logs, and hosting and vendors that sign a Business Associate Agreement. Other markets have their own rules, such as GDPR in Europe. A HIPAA compliant app development company builds these safeguards in from the start. Adding them after launch costs more and often means rework.
Platforms
A web app for providers and a mobile app for patients is a common split. Building native iOS and Android apps costs more than a cross-platform framework such as React Native or Flutter, which works well for most telemedicine products.
AI features
Teams increasingly add an AI chatbot for healthcare appointment booking and intake, visit summaries for clinicians, or reminders. These add value but need careful limits: no medical advice from the bot, and clinical questions handed to staff.
Running costs
After launch, budget for hosting, video minutes, SMS and email, AI model usage if you use it, monitoring, security updates and support. These monthly costs are easy to forget during planning.
How to scope a first version
- Pick one care pathway and one patient group.
- List the features that pathway needs and nothing more.
- Decide which systems must connect on day one.
- Confirm the compliance rules for your market.
- Ask for a fixed-scope quote broken down by feature.
That breakdown lets you move features between the first and second release to fit your budget. Our healthcare software development team quotes this way, and for hospitals we also build hospital management systems.
